On Line Registration.
1) Fill out the form below and your information will be sent to GDAHA for registration. Click on the "Submit" button at the bottom to send your information.
2) Then, follow the instructions on the right to send your registration fee to GDAHA using PayPal.
You will receive a confirmation email from email@example.com when your registration and payment are confirmed.
Mail In Registration: Click on the link to the right for the Conference Flyer and the Registration form. You can use this to mail in your registration.

NOTE: Although Nursing Home Adminstrators is listed on the registration form for contact hours, we were unable to obtain those this year due to a process change.

Step 2 of On Line Registration: Sending payment to GDAHA using PayPal.
1. Go to PayPal.com. Log in if you have an account. If you do not have an account, there will be a "SEND" button on the top middle of the screen. Follow instructions to use a credit card to send your registration fee.
2. After you are in PayPal, on the right there is a blue button icon on the upper right that says "Send". Click on "Send"
3. Send the appropriate registration fee to "email@example.com". Please enter this correctly or your payment will not go to the proper place .
Jill at GDAHA will match up your email registration from the left and your PayPal Payment, and then send you a confirmation email.
